With the encouragement of the Federal Government, EMRs are rapidly being adopted by physician practices. Many times, practices are implementing these systems without the expertise of coding professionals to guide them through creation of templates and complex coding guidelines. It is imperative that the use of an EMR not only increases the chances of providing better patient care, but also provides compliant and "audit proof" documentation.

The Office of Inspector General has indicated that they will increase scrutiny on documentation created by EMRs to identify fraudulent use of these systems - even if the fraudulent use is unintentional. They will specifically be looking at records for "cloned" documentation or for records that contain excessive documentation (complete review of systems for a problem focused visit) that may not be medically necessary based on the patient's reason for visit.
